Objective To systematically evaluate the current situation of HPV vaccination intention and its influencing factors among Chinese male college students.

Methods Relevant literature on HPV vaccination intention and its influencing factors among Chinese male college students were searched in CNKI, WanFang Data, VIP, CBM, PubMed, Web of Science, Embase and the Cochrane Library from the establishment of the databases to December 1, 2025. Two researchers independently screened the literature, extracted the data, conducted methodological quality assessment and performed Meta-analysis using Stata 18.0 software.

Results A total of 28 articles were included. The results of Meta-analysis showed that the HPV vaccination intention rate among Chinese male college students was 60.00% (95%CI: 52.00%-69.00%). The awareness of HPV and its vaccine (OR = 4.32, 95%CI: 2.01-9.27), place of origin(OR = 1.35, 95%CI: 1.14-1.60), graduate students (OR = 1.85, 95%CI: 1.08-3.15), medical major (OR = 1.49, 95%CI: 1.11-2.00), sexual orientation (OR = 4.47, 95%CI: 1.26-15.82), having heard about HPV (OR = 1.49, 95%CI: 1.23-1.80), having friends or family members with cancer (OR = 1.35, 95%CI: 1.01-1.81), having received sexual education (OR = 1.50, 95%CI: 1.27-1.78), and the affordability of the vaccine price (OR = 4.66, 95%CI: 1.54-14.11) were factors influencing the HPV vaccination intention of male college students (P < 0.05).

Conclusion The HPV vaccination intention among Chinese male college students was relatively high. There were many influencing factors. It is necessary to strengthen HPV-related health education for male college students to improve the awareness of HPV and its vaccine, thereby enhancing the HPV vaccination intention of male college students.
