Objective  To systematically evaluate the occurrence and influencing factors of social alienation among cancer survivors.

Methods  The literature about the occurrence and influencing factors of social alienation among cancer survivors were searched in CBM, CNKI, WanFang Data, VIP, Web of Science, PubMed, CINAHL, Embase, and the Cochrane Library from the establishment of the databases to September 21, 2024. Two researchers independently screened the literature, extracted data, and evaluated the quality. Stata 16.0 software was used for data analysis.

Results  A total of 48 articles were included, involving 12 633 patients. The results of Meta-analysis showed that the overall social alienation score of cancer survivors was 41.32 (95%CI: 40.18-42.46), which was above the medium level. Subgroup analysis revealed that social alienation among cancer survivors was influenced by disease type, region, education level, religious belief, monthly per capita household income, tumor stage, and treatment time. The cancer survivors who had digestive system tumors (43.64, 95%CI: 41.52-45.75), lived in Eastern China (43.72, 95%CI: 41.99-45.45), with the educational level of junior high school or less (42.47, 95%CI: 40.77- 44.18), had no religious belief (39.95, 95%CI: 39.05-40.85), and had per capita monthly household income <3  000 yuan (43.61, 95%CI: 41.24-45.98), with tumor cancer stage Ⅱ to Ⅳ (41.19, 95%CI: 40.00- 42.38), and treatment duration <1 year (43.90, 95%CI: 42.54-45.25) had a higher social alienation score (P<0.05).

Conclusion  Cancer survivors faced a higher risk of social alienation. Medical personnel should take individualized intervention measures according to the disease type, region, education level, religious belief, per capita monthly household income, tumor stage, treatment time and other risk factors to reduce the risk of social alienation in cancer survivors.
